Insurance and Fees

I am in-network with Regence BlueCross BlueShied, Moda and Pacific Source.  If I am out of network with your insurance company, I am able to provide you with an invoice that you may submit to your insurance to request out of network benefits.  Payment for services is due at the time of your visit.
